  1. Oversee the case management operations for multiple facilities within the healthcare organization.
  2. Ensure that all patients receive high-quality and compassionate care throughout their treatment.
  3. Manage costs and resources effectively to ensure efficient and cost-effective case management services.
  4. Develop and implement processes and protocols for case management to maintain compliance with regulatory standards and guidelines.
  5. Monitor and analyze data to identify opportunities for improvement in patient outcomes and cost savings.
  6. Collaborate with other healthcare leaders to develop and implement best practices for case management across the organization.
  7. Provide leadership and guidance to the case management team, including hiring, training, and performance evaluations.

  9. Represent the organization in meetings and collaborations with other healthcare providers and community organizations.
  10. Stay updated on industry trends and developments related to case management and implement changes as needed.
  11. Ensure that all case management processes and procedures are in line with the organization's mission and values.
  12. Address and resolve any issues or concerns related to case management services.
  13. Work closely with other departments, such as finance and compliance, to ensure a cohesive and integrated approach to patient care.
  14. Develop and maintain relationships with external stakeholders, including insurance companies and government agencies.
  15. Maintain accurate and up-to-date documentation and reports related to case management activities and outcomes.

Job Qualifications
  • Bachelor's Or Master's Degree In Nursing, Healthcare Administration, Or Related Field.

  • Minimum Of 5 Years Of Experience In Case Management, Utilization Management, Or Care Coordination.

  • Knowledge Of Healthcare Regulations, Including Medicare And Medicaid Guidelines.

  • Strong Leadership Skills And Experience Managing A Team Of Case Managers.

  • Excellent Communication And Interpersonal Skills, With The Ability To Collaborate With Interdisciplinary Teams And External Stakeholders.
