  1. Review and analyze medical records to ensure accuracy and completeness of clinical documentation.
  2. Collaborate with healthcare providers to obtain necessary information for complete and accurate documentation.
  3. Identify and communicate documentation deficiencies to healthcare providers and work with them to resolve any issues.
  4. Stay updated on current clinical documentation guidelines and regulations to ensure compliance.
  5. Utilize coding and classification systems to assign appropriate diagnoses and procedures.
  6. Educate healthcare providers on proper documentation practices and guidelines.
  7. Communicate effectively with a multidisciplinary team to ensure comprehensive and accurate patient records.
  8. Perform audits and quality reviews of clinical documentation to identify areas for improvement.
  9. Participate in coding and documentation training programs to further develop skills and knowledge.
  10. Maintain confidentiality and adhere to ethical standards in handling patient information.
  11. Collaborate with other departments to ensure accurate and timely billing and reimbursement.
  12. Monitor and report on key performance indicators related to clinical documentation.
  13. Contribute to the development and implementation of policies and procedures related to clinical documentation.
  14. Participate in departmental meetings and initiatives to improve patient care and outcomes.
  15. Maintain a positive and professional demeanor while interacting with patients, healthcare providers, and other team members.

Job Qualifications
  • Bachelor's Degree In Healthcare-Related Field, Such As Healthcare Administration, Nursing, Or Health Information Management.

  • Minimum Of 2 Years Experience In Clinical Documentation Improvement Or Medical Coding.

  • Certification In Clinical Documentation Improvement, Such As Certified Clinical Documentation Specialist (Ccds) Or Certified Coding Specialist (Ccs).

  • Strong Knowledge Of Icd-10 Coding Guidelines And Clinical Documentation Requirements.

  • Excellent Analytical And Critical Thinking Skills, With The Ability To Identify And Resolve Documentation Discrepancies And Coding Errors.
