  1. Conduct thorough reviews of patient medical records to ensure accuracy and completeness of clinical documentation.
  2. Collaborate with healthcare professionals, including physicians, nurses, and coding specialists, to obtain and clarify necessary information for medical record documentation.
  3. Utilize knowledge of medical terminology and coding to accurately reflect patient conditions and treatments in medical records.
  4. Identify and report any discrepancies or gaps in documentation to healthcare providers for timely correction.
  5. Analyze medical records for compliance with regulatory and billing requirements.
  6. Educate healthcare providers on proper documentation practices and communicate any changes in documentation guidelines.
  7. Stay updated on changes in coding and documentation guidelines to ensure accurate and compliant medical record documentation.
  8. Assist in developing documentation improvement strategies and initiatives to enhance patient care and support accurate coding and billing practices.
  9. Collaborate with coding and billing teams to resolve any coding or documentation issues that may arise.
  10. Maintain confidentiality and adhere to ethical standards when handling patient information.
  11. Participate in training and mentoring new clinical documentation specialists.
  12. Foster a positive and collaborative work environment by effectively communicating and working with team members and other healthcare professionals.
  13. Meet productivity and quality standards set by the organization.
  14. Continuously evaluate and improve documentation processes to ensure efficient and accurate medical record documentation.
  15. Demonstrate a commitment to providing high-quality patient care and promoting patient satisfaction.

Job Qualifications
  • Bachelor's Degree In Healthcare Administration, Nursing, Or Related Field.

  • Minimum Of 2-3 Years Of Experience In Clinical Documentation Improvement, Preferably In A Hospital Setting.

  • Strong Understanding Of Icd-10 Coding And Guidelines.

  • Knowledge Of Regulatory Requirements And Compliance Related To Clinical Documentation.

  • Excellent Communication And Interpersonal Skills, With The Ability To Work Collaboratively With Healthcare Providers And Staff.
