  • Develop and implement strategies to promote and support the health and well-being of the community
  • Conduct outreach and engage with community members to understand their health needs and concerns

  • Collaborate with healthcare providers and community organizations to coordinate and deliver health programs and services
  • Maintain accurate and up-to-date records of community interactions and services provided
  • Participate in community events and meetings to represent the organization and promote population health initiatives
  • Identify and address social determinants of health that may impact the overall well-being of the community
  • Advocate for underserved and marginalized populations to ensure equal access to health resources and services
  • Continuously evaluate and assess the effectiveness of population health programs and make recommendations for improvement
  • Stay informed on current trends and developments in population health to inform decision-making and program development.

Job Qualifications
  • High School Diploma Or Ged: A Minimum Educational Requirement For This Position Is A High School Diploma Or Ged. This Demonstrates Basic Academic Skills And Knowledge.

  • Previous Experience In Community Health Work: Previous Experience In Community Health Work Or Related Field Is Preferred. This Shows The Candidate's Understanding Of Health-Related Issues And Their Ability To Work With Diverse Populations.

  • Knowledge Of Population Health Principles: A Strong Understanding Of Population Health Principles, Including Social Determinants Of Health, Health Disparities, And Health Promotion Strategies, Is Essential For This Role.

  • Strong Communication And Interpersonal Skills: Effective Communication And Interpersonal Skills Are Necessary For Building Relationships With Community Members, Healthcare Providers, And Other Stakeholders In Order To Promote And Advocate For Population Health.

  • Bilingual Or Multilingual Skills: Fluency In More Than One Language, Particularly Spanish, Is Highly Desirable For This Role As It Allows For Effective Communication With Diverse Populations And Can Help Bridge Language Barriers In Healthcare Settings.
