
Intensive Care Coordinator - Utah County

- Develop and implement individualized care plans for individuals with complex medical needs.
- Collaborate with healthcare providers, social workers, and other members of the care team to ensure comprehensive and coordinated care.
- Conduct thorough assessments of each individual's needs and create appropriate interventions.
- Monitor and evaluate the effectiveness of care plans and make necessary adjustments.
- Communicate regularly with individuals and their families to provide support and address any concerns or questions.
- Maintain accurate and up-to-date documentation of all interactions and interventions.
- Stay current on industry standards and best practices for intensive care coordination.
- Advocate for individuals and families to ensure they receive the best possible care.
- Coordinate and facilitate communication between all involved parties, including healthcare providers, insurance companies, and community resources.
- Attend and actively participate in team meetings and trainings.
- Provide emotional support to individuals and families during difficult times.
- Maintain confidentiality and adhere to all HIPAA regulations.
- Proactively identify and address any potential issues or barriers to care.
- Collaborate with community resources to provide additional support and resources to individuals and families.
- Adhere to all organizational policies and procedures.
Strong Communication And Interpersonal Skills.
Bachelor's Degree In Social Work, Psychology, Or Related Field.
Minimum Of 2 Years Experience Working With Individuals With Mental Illness Or Developmental Disabilities.
Knowledge Of Utah County's Mental Health And Developmental Disability Systems.
Ability To Work Independently And Collaboratively With A Team.
