- Conduct initial assessments of individuals and families to determine their needs and eligibility for services.
- Develop and implement treatment plans to address identified needs and goals.
- Connect individuals with appropriate community resources, such as housing assistance, healthcare services, and job training programs.
- Provide ongoing support and guidance to clients, including crisis intervention and case management.
- Collaborate with other agencies and organizations to coordinate services and ensure the best possible outcomes for clients.
- Maintain accurate and up-to-date client records and documentation.
- Stay current on social services policies and regulations to ensure compliance and effective service delivery.
- Advocate for clients and their needs within the community and with other service providers.
- Conduct outreach and education to raise awareness of available services and resources.
- Participate in team meetings and trainings to enhance knowledge and skills.
- Handle confidential information with sensitivity and maintain professional boundaries.
- Demonstrate a compassionate and non-judgmental approach to working with individuals from diverse backgrounds.
- Comply with ethical standards and codes of conduct in all interactions and decision-making processes.
- Represent the State of Oregon in a professional and positive manner at all times.
Bachelor's Degree In Social Work, Psychology, Or A Related Field.
Minimum Of Two Years Of Experience Working In A Human Services Setting.
Knowledge Of State And Federal Laws And Regulations Related To Human Services Programs.
Strong Communication And Interpersonal Skills, With The Ability To Work Effectively With Diverse Populations.
Experience With Case Management, Crisis Intervention, And Community Resource Referrals.
