Job Description

The State of Oregon is seeking a compassionate and motivated individual to join its Human Services Case Manager team. As the Human Services Case Manager, you will be the primary point of contact for clients and their families, providing guidance and support to ensure access to essential services. You will also be responsible for developing and managing individual care plans, advocating for clients' rights, and connecting them to community resources. The ideal candidate for this position will have excellent communication and interpersonal skills, the ability to work independently, and experience with case management in a social service setting. Responsibilities:

  1. Develop and manage individual care plans for clients and their families
  2. Advocate for clients' rights and ensure access to essential services
  3. Connect clients to community resources
  4. Maintain effective communication with clients and their families
  5. Provide guidance and support to clients in a compassionate and empathetic manner
  6. Monitor and assess clients' progress
  7. Prepare reports and track client data
  8. Attend meetings with other social service providers
  9. Engage in outreach activities to promote services
  10. Remain up-to-date on relevant regulations and policies
