  1. Develop and implement strategies to improve the overall health and well-being of rural communities in the state of Massachusetts.
  2. Lead and oversee all population health initiatives and programs targeted towards rural populations.
  3. Collaborate with other healthcare organizations, community leaders, and stakeholders to identify and address the unique health challenges faced by rural residents.
  4. Conduct research and data analysis to identify health disparities and develop evidence-based solutions to promote health equity in rural areas.

  6. Develop and manage budgets for population health programs and initiatives.
  7. Provide guidance and support to community health workers and other healthcare personnel working in rural areas.
  8. Monitor and evaluate the effectiveness of population health programs and make necessary adjustments to ensure their success.
  9. Advocate for the needs of rural populations at local, state, and national levels.
  10. Stay updated on current trends and best practices in rural population health to continually improve and innovate strategies and programs.

Job Qualifications
  • Bachelor's Degree In Public Health, Health Administration, Or A Related Field.

  • Minimum Of 3 Years Of Experience In Population Health Management Or Community Health In A Rural Setting.

  • Knowledge Of Rural Health Disparities And Ability To Develop Strategies To Address Them.

  • Experience With Data Analysis And Program Evaluation To Identify And Prioritize Health Needs In Rural Communities.

  • Strong Communication And Collaboration Skills To Engage With Local Stakeholders And Community Members In Developing And Implementing Population Health Initiatives.
