  1. Conduct eligibility assessments and determine appropriate services for individuals and families in need.
  2. Develop and maintain relationships with community organizations and resources to support clients.
  3. Provide case management services to ensure clients are receiving necessary support and resources.
  4. Advocate for clients' rights and needs within the organization and in the community.
  5. Collaborate with a multidisciplinary team to develop and implement individualized service plans.
  6. Maintain accurate and up-to-date records of client interactions, progress, and services provided.
  7. Conduct home visits and outreach to identify and support at-risk populations.
  8. Stay informed about current social work principles, policies, and best practices.
  9. Provide crisis intervention and support for clients in emergency situations.
  10. Attend trainings and meetings to enhance knowledge and skills related to social work.
  11. Communicate effectively with clients, colleagues, and other professionals to ensure coordinated and effective services.
  12. Adhere to all ethical standards and guidelines in carrying out job responsibilities.
  13. Participate in program evaluation and quality improvement initiatives to ensure the delivery of high-quality services.
  14. Uphold the agency's mission, vision, and values in all interactions and decisions.
  15. Continuously assess and reassess clients' needs and make appropriate referrals as needed.
  16. Collaborate with other agencies and organizations to coordinate services and resources for clients.
  17. Provide education and support to clients and their families to promote self-sufficiency and independence.
  18. Maintain confidentiality and respect client privacy in all interactions and documentation.
  19. Conduct outreach and awareness activities to promote the program and its services.
  20. Perform any other duties as assigned by the supervisor to support the overall mission of the organization.

Job Qualifications
  • Bachelor's Degree In Social Work Or Related Field From An Accredited University.

  • Current Licensure As A Licensed Social Worker (Lsw) Or Equivalent In The State Of Massachusetts.

  • Minimum Of 2 Years Of Experience In A Social Work Setting, Preferably In Child Welfare Or Family Services.

  • Knowledge Of State And Federal Laws And Regulations Related To Social Work, Including Child Protection And Family Services.

  • Strong Communication And Interpersonal Skills, With The Ability To Effectively Work With Diverse Populations And Collaborate With Multidisciplinary Teams.
