- Provide emotional and practical support to new and expecting mothers throughout their pregnancy and postpartum journey.
- Connect clients with community health resources, such as prenatal care, nutrition services, and mental health resources.
- Conduct assessments to identify clients' needs and develop personalized plans to address them.
- Educate clients on healthy pregnancy practices and provide resources for infant care and postpartum self-care.
- Collaborate with healthcare providers, social workers, and other professionals to ensure comprehensive care for clients.
- Advocate for clients' rights and needs within the healthcare system.
- Maintain accurate and confidential client records and documentation.
- Attend trainings and stay updated on current research and best practices in perinatal care.
- Utilize cultural competency and sensitivity when working with diverse populations.
- Participate in outreach and community events to promote awareness of available services.
- Build and maintain relationships with community organizations, healthcare providers, and other stakeholders to enhance services for clients.
- Provide crisis intervention and support in emergency situations.
- Adhere to all ethical and professional standards in the field of social work.
- Continuously evaluate and improve services to meet the changing needs of clients and the community.
- Maintain regular communication and collaboration with team members and supervisors.
Bachelor's Degree In Social Work Or Related Field From An Accredited College Or University.
Minimum Of 2 Years Of Experience In Social Work, Preferably In A Perinatal Or Maternal Health Setting.
Knowledge And Understanding Of Perinatal Mental Health, Trauma-Informed Care, And Evidence-Based Interventions.
Strong Communication And Interpersonal Skills, With The Ability To Build Rapport And Collaborate With Diverse Populations.
Experience Working In A Community-Based Or Public Health Setting, With A Strong Understanding Of The Social Determinants Of Health And Their Impact On Perinatal Outcomes.
