- Provide direct support and assistance to individuals and families in need, including but not limited to emotional support, referrals to community resources, and assistance with basic needs such as food, housing, and healthcare.
- Conduct thorough assessments of clients' needs and develop individualized plans to address those needs.
- Maintain accurate and up-to-date client records and documentation.
- Collaborate with other professionals and agencies to coordinate services and ensure the needs of clients are met.
- Conduct home visits and make referrals for follow-up services as needed.
- Keep abreast of current laws, regulations, and policies related to human services and ensure compliance.
- Act as an advocate for clients, helping them navigate the complex human services system.
- Provide crisis intervention and support to individuals and families in emergency situations.
- Communicate effectively with clients from diverse backgrounds, demonstrating cultural competence and sensitivity.
- Participate in ongoing training and professional development opportunities to enhance job performance and stay updated on best practices in the field.
- Maintain confidentiality and adhere to ethical standards in all interactions with clients.
- Demonstrate empathy, compassion, and a non-judgmental attitude towards clients.
- Work collaboratively with colleagues and supervisors to ensure a cohesive and effective team approach in providing services.
- Monitor and report any concerns or changes in clients' well-being to appropriate parties.
- Perform other duties as assigned by supervisors in support of the agency's mission and goals.
Bachelor's Degree In Social Work, Psychology, Or Related Field.
Minimum Of 2 Years Of Experience Working In A Human Services Or Related Field.
Knowledge Of State And Federal Regulations And Laws Related To Human Services.
Strong Communication And Interpersonal Skills To Effectively Work With Clients, Families, And Other Professionals.
Ability To Handle Confidential Information And Maintain Ethical Standards In All Interactions.
