
Assistant Case Manager

- Work closely with Case Managers to provide support and assistance to individuals and families in need.

- Conduct intake interviews and assessments to determine client needs.
- Assist in creating and implementing individualized care plans for clients.
- Coordinate and schedule appointments and meetings for clients.
- Communicate with external agencies and resources to obtain necessary services for clients.
- Provide emotional support and guidance to clients during difficult situations.
- Keep up-to-date with changes in policies and regulations related to social services.
- Participate in team meetings and collaborate with colleagues to ensure the best possible care for clients.
- Handle sensitive and confidential information with professionalism and discretion.
- Maintain a positive and professional demeanor when interacting with clients and colleagues.
- Prioritize tasks and manage time effectively to meet deadlines.
- Advocate for clients and their needs within the community.
- Attend training and professional development opportunities to enhance knowledge and skills.
- Serve as a liaison between clients and other service providers to ensure continuity of care.
Bachelor's Degree In Social Work, Psychology, Or A Related Field.
Prior Experience In Case Management Or Social Services, Preferably In A Government Setting.
Strong Communication And Interpersonal Skills To Effectively Interact With Clients, Colleagues, And Stakeholders.
Knowledge Of Arizona State Laws And Regulations Related To Social Services And Case Management.
Ability To Handle Sensitive And Confidential Information With Discretion And Maintain Professional Boundaries With Clients.
