1. Develop and maintain relationships with healthcare providers to promote Select Medical's services and establish a network of referral sources.
2. Serve as the primary point of contact for healthcare providers, patients, and families during the transition from acute care to Select Medical facilities.
3. Educate healthcare providers on the services and benefits offered by Select Medical, including specialized programs and treatment options.
4. Conduct on-site visits to hospitals and other healthcare facilities to identify potential patients and facilitate smooth transitions to Select Medical facilities.
5. Collaborate with clinical teams to assess patient needs and ensure appropriate placement within Select Medical facilities.
6. Communicate effectively with patients, families, and healthcare providers to address any concerns or questions and provide updates on patient progress.
7. Monitor and track patient referrals and admissions, and report this information to management.
8. Stay current on industry trends, competitor offerings, and changes in healthcare regulations that may impact Select Medical's services.
9. Act as a liaison between Select Medical facilities and external stakeholders, including insurance companies and case managers.
10. Participate in marketing and outreach efforts to promote Select Medical's services and increase brand awareness.
11. Adhere to all company policies and procedures, as well as ethical and legal standards, when interacting with patients, families, and healthcare providers.
12. Maintain accurate and thorough documentation of all interactions and activities related to patient referrals and admissions.
13. Collaborate with other healthcare professionals, including physicians, nurses, and therapists, to ensure coordinated and high-quality patient care.
14. Continuously evaluate and improve processes to enhance the patient experience and increase efficiency.
