  1. Coordinate and manage the care of patients to ensure their needs are met in a timely and efficient manner.
  2. Communicate effectively with patients, their families, and healthcare professionals to gather and share information related to patient care.
  3. Utilize strong organizational skills to manage and prioritize a caseload of patients.
  4. Demonstrate a thorough understanding of medical terminology and procedures to effectively assess and manage patient care.
  5. Maintain accurate and detailed patient records and documentation.
  6. Collaborate with other members of the healthcare team to develop and implement individualized patient care plans.
  7. Advocate for patients and their families to ensure they receive the best possible care and support.
  8. Stay up-to-date on industry standards and best practices related to case management.
  9. Participate in multidisciplinary team meetings to discuss patient progress and develop treatment plans.
  10. Maintain confidentiality and adhere to ethical standards in all aspects of the job.
  11. Display compassion and empathy towards patients and their families during difficult and challenging times.
  12. Continuously strive to improve patient outcomes and satisfaction.
  13. Adhere to all relevant laws, regulations, and company policies.
  14. Attend training and educational opportunities to enhance skills and knowledge.
  15. Serve as a positive representative of Select Medical and uphold the company's mission and values.

Job Qualifications
  • Bachelor's Degree In Social Work, Nursing, Or Related Field

  • Minimum Of 2 Years Experience In Case Management Or Related Healthcare Field

  • Knowledge Of Healthcare Regulations And Laws, Including Hipaa And Medicare/Medicaid

  • Excellent Communication And Interpersonal Skills, With The Ability To Work With Diverse Populations

  • Strong Organizational And Time Management Skills, With The Ability To Prioritize And Multitask Effectively
