Social Work Care Coordinator

The ideal candidate will have a minimum of 2 years of experience in social work, care coordination, or a related field, and a Bachelor’s degree in social work or a related field. The successful applicant should demonstrate a commitment to excellence in patient care and an understanding of the healthcare system. This individual will be a strong advocate for patients, coordinate care plans, and collaborate with a variety of professionals to ensure the highest quality of care for our patients. Responsibilities:

• Develop individualized care plans for patients, taking into account their unique needs and preferences
• Act as an advocate for patients, ensuring that their needs are met, and that they receive the best possible care
• Collaborate with other healthcare professionals to coordinate care plans and ensure the highest quality of care
• Monitor patients’ progress and adjust care plans as needed
• Assist patients in understanding their medical conditions and available treatment options
• Educate patients and their families on the healthcare system, available resources, and insurance coverage
• Attend and document all patient appointments and follow-up visits
• Maintain accurate and detailed patient records
• Provide emotional support to patients and their families
