
Discharge Planning Assistant
Renown Health is looking for a highly organized and reliable Discharge Planning Assistant to join our team. We need an individual who is passionate about patient care and is dedicated to providing the highest quality of service. The ideal candidate will possess excellent communication and organizational skills, as well as the ability to work effectively with a team in a fast-paced environment. The Discharge Planning Assistant plays a vital role in providing a smooth transition from hospital to home for our patients. This position requires the ability to work independently and to prioritize multiple tasks efficiently. The successful applicant will assist with discharge planning, provide education to patients and families, and coordinate with other departments to ensure a successful discharge process. Responsibilities:
- Assist with discharge planning and provide patient and family education.

- Communicate effectively with patients, families, and other healthcare personnel.
