
IOP Transition Representative

- Coordinate and manage the transition of patients from inpatient to outpatient care settings.
- Communicate effectively with patients, healthcare providers, and other members of the healthcare team to ensure a smooth and timely transition.
- Utilize strong organizational skills and attention to detail to accurately track patient progress and ensure all necessary documentation is completed.
- Work independently and as part of a team to meet transition goals and timelines.
- Collaborate with various departments within Quest Diagnostics to ensure a coordinated and efficient transition process.
- Provide exceptional customer service to patients and healthcare providers, addressing any questions or concerns that may arise during the transition process.
- Maintain up-to-date knowledge of healthcare regulations and guidelines to ensure compliance throughout the transition process.
- Use critical thinking and problem-solving skills to identify and resolve any issues or barriers to the transition process.
- Participate in training and development opportunities to continuously improve skills and knowledge.

Bachelor's Degree In Business Administration Or A Related Field.
Minimum Of 2 Years Experience In Customer Service Or Sales.
Strong Communication And Interpersonal Skills.
Knowledge Of Healthcare Industry And Understanding Of Iop (Independent Physician Organization) Model.
Ability To Travel And Work Independently In A Fast-Paced Environment.
