  1. Oversee the social services department and ensure the provision of high-quality care to patients and their families.
  2. Develop and implement policies and procedures for the social services department in alignment with the organization's mission and values.
  3. Lead and supervise a team of social workers, providing guidance, support, and performance evaluations.
  4. Collaborate with other departments and healthcare professionals to coordinate and integrate social services into patient care plans.
  5. Conduct assessments and develop individualized care plans for patients, addressing their social, emotional, and psychological needs.
  6. Serve as a liaison between patients, families, and community resources to facilitate access to necessary services and support.
  7. Monitor and evaluate the effectiveness of social services programs and make recommendations for improvements.
  8. Ensure compliance with all applicable laws, regulations, and ethical standards related to social work and patient care.
  9. Maintain accurate and confidential records of patient interactions, progress, and services provided.
  10. Participate in interdisciplinary team meetings and provide input on patient care and discharge planning.
  11. Stay updated on current social work practices, trends, and research to continuously improve the quality of care provided.
  12. Foster a positive and supportive work environment, promoting teamwork, collaboration, and professional growth among department staff.
  13. Serve as a role model for compassionate and ethical behavior, promoting a patient-centered approach to care.
  14. Communicate effectively with patients, families, and staff, providing education and support as needed.
  15. Adhere to the organization's code of conduct and ethics, maintaining professionalism and integrity in all interactions.

Job Qualifications
  • Bachelor's Or Master's Degree In Social Work Or A Related Field.

  • Minimum Of 5 Years Of Experience In Social Services, Preferably In A Healthcare Setting.

  • Knowledge Of Federal And State Regulations And Guidelines Related To Social Services In Long-Term Care Facilities.

  • Strong Leadership Skills And Experience Managing A Team Of Social Workers.

  • Excellent Communication And Interpersonal Skills, With The Ability To Collaborate With Other Departments And Community Resources To Meet The Needs Of Residents.
