  1. Develop and implement social services programs and initiatives to meet the needs of our patients and residents.
  2. Oversee and coordinate all aspects of social services within the facility, including staffing, budgeting, and resource allocation.
  3. Conduct assessments to determine the social, emotional, and psychological needs of patients and residents.
  4. Collaborate with other departments to ensure a comprehensive and integrated approach to patient care.
  5. Provide support and counseling to patients and families, addressing any concerns or issues that may arise.
  6. Maintain accurate and up-to-date records and documentation of all social services activities.
  7. Ensure compliance with all applicable laws, regulations, and policies related to social services.
  8. Stay informed on the latest developments and best practices in social services in order to continuously improve and enhance the quality of care provided.
  9. Facilitate communication between patients, families, and healthcare team members to ensure a positive and supportive environment.
  10. Participate in interdisciplinary care planning and team meetings to provide input and recommendations for improving patient outcomes.
  11. Serve as a liaison between the facility and community resources, building and maintaining strong relationships to enhance the care and services available to our patients and residents.
  12. Provide guidance and mentorship to social services staff, promoting a positive and supportive work environment.
  13. Collaborate with other healthcare professionals and organizations to advocate for the needs of our patients and residents.
  14. Participate in quality improvement initiatives to evaluate and improve the effectiveness of social services programs and resources.
  15. Maintain confidentiality and adhere to ethical standards in all interactions with patients, families, and staff.

Job Qualifications
  • Bachelor's Or Master's Degree In Social Work, Psychology, Or Related Field

  • Minimum Of 5 Years Experience In A Leadership Role In Social Services Or Healthcare Setting

  • Knowledge Of State And Federal Regulations And Policies Related To Social Services

  • Strong Communication And Interpersonal Skills To Effectively Collaborate With Interdisciplinary Team Members, Residents, And Families

  • Ability To Develop And Implement Programs And Services To Meet The Psychosocial Needs Of Residents And Their Families.
