Social Services Assistant

You must be able to work well independently and as part of a team, and have strong problem-solving and organizational skills. Responsibilities:

1. Assist multi-disciplinary team of healthcare professionals in providing high quality care to patients.
2. Develop and implement strategies that promote independence and well-being for residents and their families.
3. Provide emotional support and guidance to patients and their families.
4. Collaborate with other staff members to ensure patients receive comprehensive care.
5. Advocate for patient rights and ensure patient confidentiality.
6. Monitor and evaluate patient progress and document care plans.
7. Respond to any patient inquiries and concerns in a timely manner.
8. Work with other departments to ensure patient needs are met.
9. Participate in team meetings and other related activities.
10. Maintain up-to-date knowledge on relevant social services and healthcare topics.
