In this position, you will play a crucial role in ensuring the highest level of care for our residents by coordinating and overseeing the Minimum Data Set (MDS) process.   1. Coordinate and oversee the Minimum Data Set (MDS) process to ensure accurate and timely completion for all residents.
  2. Ensure compliance with state and federal regulations related to MDS and long-term care.
  3. Collaborate with interdisciplinary team members to accurately assess and document residents' clinical needs.
  4. Develop and maintain care plans based on MDS assessments, resident goals, and industry standards.
  5. Train and educate staff on MDS process and procedures.
  6. Monitor and track changes in residents' conditions and update care plans accordingly.
  7. Review and analyze MDS data to identify areas for improvement and implement quality improvement initiatives.
  8. Communicate effectively with residents, families, and healthcare team members to ensure the highest level of care.
  9. Serve as a resource for staff regarding MDS and long-term care regulations.
  10. Participate in meetings and committees related to MDS and quality improvement.
  11. Maintain accurate and up-to-date resident records and documentation.
  12. Provide leadership and support to nursing staff in achieving optimal resident outcomes.
  13. Collaborate with other departments to ensure seamless coordination of care.
  14. Stay updated on changes to MDS and long-term care regulations and implement changes as needed.
  15. Demonstrate compassion, empathy, and respect for residents and their families.

Job Qualifications
  • Active Rn License: The Candidate Must Possess A Valid And Active License As A Registered Nurse In The State Where The Facility Is Located.

  • Minimum Of 3 Years Of Experience In Mds Coordination: The Ideal Candidate Must Have At Least 3 Years Of Experience In Mds Coordination In A Skilled Nursing Facility Or Long-Term Care Setting.

  • Knowledge Of Mds Software And Regulations: The Candidate Should Have A Strong Understanding Of Mds Software And Regulations, Including The Rai Process And Medicare Guidelines.

  • Strong Communication And Organizational Skills: As An Mds Coordinator, The Candidate Will Be Responsible For Coordinating And Communicating With Various Healthcare Professionals, So Strong Communication And Organizational Skills Are Essential.

  • Attention To Detail And Accuracy: The Mds Coordinator Is Responsible For Completing And Submitting Accurate Mds Assessments, Making It Crucial For The Candidate To Have Excellent Attention To Detail And Accuracy In Their Work.
