We are currently seeking a compassionate and experienced Registered Nurse to join our team as an MDS Coordinator. In this role, you will be responsible for coordinating and completing Minimum Data Set (MDS) assessments for our residents, ensuring accuracy and compliance with state and federal regulations.   1. Coordinate and complete MDS assessments for all residents, ensuring accuracy and compliance with state and federal regulations.
  2. Develop and maintain a comprehensive understanding of MDS guidelines and protocols.
  3. Collaborate with interdisciplinary team members to gather necessary information for MDS assessments.
  4. Ensure timely submission of MDS assessments to appropriate agencies.
  5. Review and analyze resident data to identify potential care needs and develop individualized care plans.
  6. Communicate with healthcare providers and family members to obtain necessary information for MDS assessments.
  7. Stay up-to-date on changes in MDS guidelines and regulations and implement necessary changes in assessment processes.
  8. Serve as a resource for staff regarding MDS assessments and related documentation.
  9. Conduct audits to ensure accuracy and completeness of MDS assessments and documentation.
  10. Participate in quality improvement initiatives related to MDS assessments and resident care.
  11. Maintain confidentiality of resident information and adhere to HIPAA guidelines.
  12. Provide guidance and training to staff members on MDS assessments and documentation.
  13. Serve as a liaison between the facility and outside agencies during MDS assessments.
  14. Attend required trainings and meetings to stay current on industry standards and regulations.
  15. Adhere to facility policies and procedures and promote a positive work environment.

Job Qualifications
  • Active Registered Nurse (Rn) License In The State Of Employment

  • Minimum Of 2 Years Of Experience In Long-Term Care Or Skilled Nursing Facility Setting

  • Knowledge Of Medicare And Medicaid Regulations And Reimbursement Guidelines

  • Strong Assessment And Documentation Skills

  • Experience With Mds Software And Electronic Medical Records Systems.
