
Registered Nurse - MDS Coordinator

- Conduct MDS assessments for all residents in accordance with state and federal regulations.
- Coordinate with interdisciplinary team to ensure accurate and timely completion of MDS assessments.
- Review and analyze resident medical records to identify and document pertinent information for MDS assessments.
- Ensure all MDS assessments are coded and submitted accurately and in a timely manner.
- Collaborate with healthcare team to develop and implement individualized care plans based on MDS assessment findings.
- Provide education and training to staff regarding MDS assessment process and regulations.
- Monitor and track changes in resident conditions and ensure appropriate updates are made to MDS assessments.
- Serve as a liaison between facility and external agencies for MDS related matters.
- Review and analyze facility data to identify areas for improvement in MDS processes and resident care.
- Ensure compliance with all state and federal regulations related to MDS assessments.
- Participate in quality assurance and performance improvement initiatives related to MDS assessments.
- Communicate effectively with residents, families, and healthcare team members regarding MDS assessment findings and care plans.
- Maintain accurate and up-to-date documentation and records related to MDS assessments.
- Stay current on changes and updates to MDS assessment guidelines and regulations.
- Foster a positive and collaborative work environment to promote quality resident care and regulatory compliance.
Current Rn Licensure: The Primary Qualification For A Registered Nurse - Mds Coordinator At Pruitthealth Is A Current And Valid Rn License. This Ensures That The Candidate Has Completed The Necessary Education And Training To Provide Skilled Nursing Care And Is Authorized To Practice In The State.
Minimum Of 2 Years Of Clinical Experience: Pruitthealth Requires A Minimum Of 2 Years Of Clinical Experience As An Rn, Preferably In A Long-Term Care Or Skilled Nursing Facility Setting. This Experience Provides The Necessary Foundation For Understanding The Complexities Of Resident Care And The Skills Needed To Assess And Document Resident Needs Accurately.
Mds Certification: A Registered Nurse - Mds Coordinator Must Have A Current Mds Certification Through The American Association Of Nurse Assessment Coordinators (Aanac). This Certification Demonstrates A Thorough Understanding Of The Minimum Data Set (Mds) And Ensures That The Coordinator Is Up-To-Date With The Latest Regulations And Guidelines.
Knowledge Of Medicare And Medicaid Regulations: Pruitthealth Requires A Strong Understanding Of Medicare And Medicaid Regulations As They Pertain To Long-Term Care And Skilled Nursing Facilities. The Mds Coordinator Must Ensure That All Documentation And Assessments Are Compliant With These Regulations To Ensure Proper Reimbursement And Resident Care.
Strong Communication And Organizational Skills: As An Mds Coordinator, Effective Communication And Organization Are Essential.
