Nurse Navigator

Responsibilities:

1. Provide comprehensive patient care coordination and support services to individuals and their families.
2. Provide education and support to ensure patients receive the highest quality of care.
3. Identify and address patient and family needs and concerns.
4. Monitor patient progress and coordinate with other healthcare providers to ensure continuity of care.
5. Maintain up-to-date patient records and other documentation.
6. Ensure compliance with applicable regulatory requirements and best practices.
7. Provide exceptional customer service to patients and their families.
8. Work independently and as part of a multi-disciplinary team.
9. Actively participate in medical team meetings and provide feedback.
10. Develop and implement strategies to improve patient outcomes.
