- Coordinate and manage the Minimum Data Set (MDS) process for patients in skilled nursing and rehabilitation facilities.
- Ensure accurate and timely completion of MDS assessments for all patients.
- Collaborate with interdisciplinary team members to gather information and complete MDS assessments.
- Review and analyze patient data to ensure compliance with federal and state regulations.
- Maintain up-to-date knowledge of MDS regulations and requirements.
- Communicate with physicians, nurses, and other healthcare professionals to ensure accurate and comprehensive patient assessments.
- Develop and implement care plans based on MDS assessments and patients' individual needs.
- Monitor and track patient progress and make necessary adjustments to care plans as needed.
- Train and educate staff on MDS processes, regulations, and documentation requirements.
- Ensure confidentiality and privacy of patient information in accordance with HIPAA regulations.
- Participate in quality improvement initiatives to enhance patient care and outcomes.
- Adhere to company policies and procedures, as well as state and federal regulations.
- Provide exceptional patient care and maintain a positive and professional attitude at all times.
- Continuously strive to improve processes and procedures related to MDS coordination.

Valid Lpn License: The First And Most Important Qualification For A Mds Coordinator Lpn Position At Pruitthealth Is A Valid Lpn License. This Ensures That The Candidate Has Completed The Necessary Training And Possesses The Required Knowledge And Skills To Perform The Duties Of A Mds Coordinator.
Minimum Of 2 Years Of Experience In Long-Term Care: Pruitthealth Requires Applicants For The Mds Coordinator Lpn Position To Have A Minimum Of 2 Years Of Experience In A Long-Term Care Setting. This Experience Provides A Strong Foundation For Understanding The Needs And Requirements Of Residents In Long-Term Care.
Knowledge Of Mds And Rai Process: The Mds Coordinator Lpn At Pruitthealth Is Responsible For Completing And Submitting Accurate Mds Assessments. Therefore, Candidates Should Have A Thorough Understanding Of The Mds And Rai Process And Be Familiar With The Cms Guidelines.
