MDS Coordinator LPN

1. Coordinate and manage the Minimum Data Set (MDS) process for patients in skilled nursing and rehabilitation facilities.
2. Ensure accurate and timely completion of MDS assessments for all patients.
3. Collaborate with interdisciplinary team members to gather information and complete MDS assessments.
4. Review and analyze patient data to ensure compliance with federal and state regulations.
5. Maintain up-to-date knowledge of MDS regulations and requirements.
6. Communicate with physicians, nurses, and other healthcare professionals to ensure accurate and comprehensive patient assessments.
7. Develop and implement care plans based on MDS assessments and patients' individual needs.
8. Monitor and track patient progress and make necessary adjustments to care plans as needed.
9. Train and educate staff on MDS processes, regulations, and documentation requirements.
10. Ensure confidentiality and privacy of patient information in accordance with HIPAA regulations.
11. Participate in quality improvement initiatives to enhance patient care and outcomes.
12. Adhere to company policies and procedures, as well as state and federal regulations.
13. Provide exceptional patient care and maintain a positive and professional attitude at all times.
14. Continuously strive to improve processes and procedures related to MDS coordination.
15. Collaborate with other healthcare professionals to ensure a comprehensive and coordinated approach to patient care.
16. Attend and actively participate in team meetings and educational opportunities.
17. Act as a liaison between patients, families, and healthcare providers to address any concerns or questions related to the MDS process.
18. Uphold the mission and values of PruittHealth by providing compassionate and quality care to patients.
19. Maintain accurate and organized documentation related to MDS assessments, care plans, and other relevant information.
20. Serve as a resource and mentor for other team members, sharing knowledge and expertise to improve overall performance.
