LMSW Hospice Social Worker

PruittHealth is looking for a compassionate and dedicated Licensed Medical Social Worker (LMSW) to join our Hospice team! As an LMSW Hospice Social Worker, you will serve as an integral member of our hospice team, providing clinical services and support to patients and their families. This position requires an individual with excellent communication skills, a passion for helping others, and the ability to provide social work services in a hospice setting.The ideal candidate must have a valid LMSW license and a minimum of two years of experience in a hospice or home health setting. Responsibilities:

1. Provide clinical services and support to patients and families in a hospice setting
2. Ensure compliance with hospice medical benefits and regulations
3. Develop and implement social work plans for patients
4. Maintain accurate and complete medical records and documentation
5. Assist patients and their families in navigating the hospice system
6. Communicate effectively with other members of the hospice team
7. Support patients and families through difficult times
8. Provide guidance and support to families in crisis
9. Monitor patient progress and adjust social work plans accordingly
10. Educate patients and families about hospice services and resources
