LMSW Hospice Social Worker

PruittHealth is looking for an experienced, compassionate, and dedicated Licensed Master Social Worker (LMSW) to join our Hospice Social Work team. In this role, you will provide expertise, support, and counseling to our hospice patients and their families as they navigate end-of-life decisions. You will be responsible for assessing and managing the psychosocial needs of patients and their families. This position requires someone with a strong background in medical social work, excellent interpersonal skills, and the ability to work independently and as part of a multidisciplinary team. The ideal candidate has experience in hospice or home health social work, is comfortable working in a fast-paced environment, and is committed to providing compassionate care to our patients and their families.

Responsibilities:

1. Provide psychosocial support to hospice patients and their families.
2. Assess and manage the psychosocial needs of patients and their families.
3. Develop and implement individualized care plans.
4. Educate patients and families on end-of-life decisions.
5. Establish and maintain positive working relationships with patients, families, and other members of the health care team.
6. Facilitate referrals and collaborate with community resources.
7. Provide crisis intervention and grief counseling.
8. Maintain accurate documentation and records of patient services.
9. Participate in staff meetings and other professional development activities.
