Job Description

We’re looking for an experienced Licensed Master Social Worker (LMSW) to join our team at PruittHealth Hospice. As a social worker, you will serve as a valuable resource to our patients and their families, providing emotional support and resources to those facing terminal illnesses. To be successful in this role, you will need to be an empathetic listener, highly organized, and have strong interpersonal skills. You should have an in-depth knowledge of hospice, end-of-life care, and the resources available in the local community. Your understanding of the unique challenges facing terminally ill patients and their families will be a key factor in helping us fulfill our commitment to providing the highest quality of care.We’re seeking a passionate, dedicated individual who is motivated to make a difference in the lives of those in their care. Responsibilities:

  1. Provide emotional support and resources to patients and families facing terminal illnesses.
  2. Listen empathetically to patients and their families.
  3. Maintain patient records and provide updates as needed.
  4. Develop a plan of care to meet the needs of the patient and their families.
  5. Communicate with other members of the healthcare team to ensure the best quality of service.
  6. Assist patients and families in understanding end-of-life care options.
  7. Develop and maintain relationships with local resources and social services.
  8. Provide crisis intervention services as needed.
  9. Participate in continuing education programs to stay current on best practices.
  10. Participate in team meetings and provide input on strategies for improving care.
