
Licensed Practical Nurse - MDS Coordinator

- Conduct timely and accurate MDS assessments for patients in accordance with state and federal regulations.
- Collaborate with interdisciplinary team members to develop individualized care plans for patients.
- Review medical records and documentation to ensure accuracy and completeness.
- Monitor and track changes in patient conditions and update care plans accordingly.
- Communicate effectively with patients, families, and caregivers to provide education and support.
- Stay up-to-date on MDS regulations and ensure compliance with all guidelines.
- Utilize critical thinking skills to identify and address any discrepancies or issues with MDS assessments.
- Train and educate staff on MDS processes and regulations.
- Maintain confidentiality of patient information and adhere to HIPAA guidelines.
- Participate in quality improvement initiatives to enhance patient care and outcomes.
- Collaborate with management to track and analyze MDS data and identify areas for improvement.
- Provide support and guidance to nursing staff regarding MDS assessments.
- Maintain accurate and organized records of MDS assessments and care plans.
- Serve as a liaison between the facility and regulatory agencies regarding MDS assessments.

Valid Lpn License In The State Of Employment: A Licensed Practical Nurse (Lpn) Must Be Licensed To Practice Nursing In The State Where They Are Employed. This Ensures That They Have Met The Necessary Education And Training Requirements And Are Legally Permitted To Provide Patient Care.
Minimum Of 2 Years Of Experience In Long-Term Care: Pruitthealth Is A Long-Term Care Facility, So It Is Important That The Lpn-Mds Coordinator Has Experience Working In This Type Of Setting. A Minimum Of 2 Years Of Experience In Long-Term Care Ensures That The Candidate Is Familiar With The Unique Needs And Challenges Of This Patient Population.
Strong Knowledge Of Mds Assessments And Regulations: As An Mds Coordinator, The Lpn Will Be Responsible For Conducting And Overseeing Minimum Data Set (Mds) Assessments For Patients. This Requires A Thorough Understanding Of Mds Guidelines And Regulations Set By The Center For Medicare And Medicaid Services (Cms).
Excellent Communication And Documentation Skills: The Lpn-Mds Coordinator Will Be Responsible For Communicating With Patients, Families, And Other Healthcare Providers, As Well As Accurately Documenting Patient Information. Strong Communication And Documentation Skills Are Essential For This Role.
Ability To Work Independently And As Part Of A Team: As An Mds Coordinator, The Lpn Will Have To Work Independently To Complete Assessments And Documentation, But Also Collaborate With Other Healthcare Professionals To Ensure Quality Care For Patients. The Ability To Work Well Both Independently And As Part Of A Team Is Crucial For Success In This Role.
