
Licensed Practical Nurse - MDS Case Mix Coordinator

- Accurately and timely completion of Minimum Data Set (MDS) assessments.
- Maintain compliance with state and federal regulations related to MDS.
- Utilize extensive knowledge of MDS guidelines to ensure accurate and thorough completion of assessments.
- Collaborate with interdisciplinary team members to gather necessary information for MDS assessments.
- Communicate effectively with residents, families, and healthcare team members regarding MDS assessments and care plans.
- Review and analyze resident medical records to identify potential discrepancies or missing information.
- Coordinate and participate in care plan meetings to discuss MDS assessments and develop individualized care plans.
- Train and educate healthcare team members on MDS guidelines, regulations, and documentation requirements.
- Serve as a resource for questions and concerns related to MDS assessments and documentation.
- Participate in quality improvement initiatives and ensure adherence to company policies and procedures.
- Maintain accurate and up-to-date records of MDS assessments and related documentation.
- Monitor and track resident care plans to ensure they are reflective of MDS assessments and accurately document resident needs.
- Identify opportunities for improvement in MDS processes and make recommendations for changes.
- Adhere to professional standards, company values, and ethical principles while performing job duties.
- Provide compassionate and high-quality care to residents in accordance with company standards and resident needs.
Current Lpn Licensure: The Candidate Must Hold A Valid Lpn License In The State Where They Will Be Working As A Mds Case Mix Coordinator. This Ensures That They Have Completed The Necessary Education And Training To Perform Nursing Duties Safely And Effectively.
Experience In Mds Coordination: The Ideal Candidate Should Have A Minimum Of 1-2 Years Of Experience Working As A Mds Coordinator In A Healthcare Facility. This Experience Demonstrates Their Understanding Of The Mds Process And Their Ability To Accurately Complete And Submit Mds Assessments.
Knowledge Of Medicare And Medicaid Regulations: The Mds Coordinator Is Responsible For Ensuring That All Mds Assessments Are Completed In Compliance With Medicare And Medicaid Regulations. Therefore, Candidates Should Have A Thorough Understanding Of These Regulations And Their Impact On The Mds Process.
