We are currently seeking a Licensed Practical Nurse (LPN) to join our team as an MDS Case Mix Coordinator. In this role, you will play a vital role in ensuring accurate and timely completion of Minimum Data Set (MDS) assessments and maintaining compliance with state and federal regulations. The ideal candidate will have a passion for providing exceptional care, strong communication skills, and extensive knowledge of MDS guidelines.   1. Accurately and timely completion of Minimum Data Set (MDS) assessments.
  2. Maintain compliance with state and federal regulations related to MDS.
  3. Utilize extensive knowledge of MDS guidelines to ensure accurate and thorough completion of assessments.
  4. Collaborate with interdisciplinary team members to gather necessary information for MDS assessments.
  5. Communicate effectively with residents, families, and healthcare team members regarding MDS assessments and care plans.
  6. Review and analyze resident medical records to identify potential discrepancies or missing information.
  7. Coordinate and participate in care plan meetings to discuss MDS assessments and develop individualized care plans.
  8. Train and educate healthcare team members on MDS guidelines, regulations, and documentation requirements.
  9. Serve as a resource for questions and concerns related to MDS assessments and documentation.
  10. Participate in quality improvement initiatives and ensure adherence to company policies and procedures.
  11. Maintain accurate and up-to-date records of MDS assessments and related documentation.
  12. Monitor and track resident care plans to ensure they are reflective of MDS assessments and accurately document resident needs.
  13. Identify opportunities for improvement in MDS processes and make recommendations for changes.
  14. Adhere to professional standards, company values, and ethical principles while performing job duties.
  15. Provide compassionate and high-quality care to residents in accordance with company standards and resident needs.

Job Qualifications
  • Current Lpn Licensure: The Candidate Must Hold A Valid Lpn License In The State Where They Will Be Working As A Mds Case Mix Coordinator. This Ensures That They Have Completed The Necessary Education And Training To Perform Nursing Duties Safely And Effectively.

  • Experience In Mds Coordination: The Ideal Candidate Should Have A Minimum Of 1-2 Years Of Experience Working As A Mds Coordinator In A Healthcare Facility. This Experience Demonstrates Their Understanding Of The Mds Process And Their Ability To Accurately Complete And Submit Mds Assessments.

  • Knowledge Of Medicare And Medicaid Regulations: The Mds Coordinator Is Responsible For Ensuring That All Mds Assessments Are Completed In Compliance With Medicare And Medicaid Regulations. Therefore, Candidates Should Have A Thorough Understanding Of These Regulations And Their Impact On The Mds Process.

  • Attention To Detail: Mds Assessments Require A High Level Of Accuracy And Attention To Detail. Candidates Should Possess Strong Organizational Skills And The Ability To Analyze And Interpret Complex Data. This Ensures That All Mds Assessments Are Completed Accurately And On Time.

  • Communication And Interpersonal Skills: As A Mds Case Mix Coordinator, The Candidate Will Be Responsible For Collaborating With Other Healthcare Professionals, Residents, And Their Families.
