  1. Administer medication and treatments as prescribed by the physician.
  2. Monitor patients' health and report any changes or concerns to the physician.
  3. Develop and implement individualized care plans for each patient.
  4. Provide emotional and physical support to patients and their families.
  5. Educate patients and their families on their conditions and how to manage them at home.
  6. Communicate effectively with the interdisciplinary team to ensure coordinated and comprehensive care.
  7. Document patient care accurately and timely.
  8. Follow all policies, procedures, and regulations to ensure quality care and compliance.
  9. Maintain a safe and clean environment for patients.
  10. Continuously assess and evaluate patient's progress and adjust care plan accordingly.
  11. Collaborate with other healthcare professionals to provide holistic care.
  12. Attend trainings and keep up-to-date with best practices and medical advancements.
  13. Provide guidance and supervision to Certified Nursing Assistants (CNAs).
  14. Maintain confidentiality and adhere to ethical standards.
  15. Foster a positive and compassionate relationship with patients and their families.

Job Qualifications
  • Current Lpn Licensure: Candidates For A Lpn Home Health Job At Pruitthealth Must Possess A Valid And Current Lpn License In The State Where They Will Be Working. This Demonstrates That The Candidate Has Met The Necessary Educational And Training Requirements To Provide Quality Nursing Care.

  • Experience In Home Health Or Related Setting: Candidates Should Have Prior Experience Working In A Home Health Or Similar Setting, Such As Hospice Or Long-Term Care. This Experience Shows That The Candidate Is Familiar With The Unique Challenges And Responsibilities Of Providing Care In A Patient's Home.

  • Strong Communication Skills: As An Lpn Home Health Nurse, Effective Communication Is Essential In Coordinating Care With Patients, Their Families, And Other Healthcare Professionals. Candidates Should Have Strong Verbal And Written Communication Skills To Ensure Clear And Accurate Communication.

  • Knowledge Of Home Health Regulations: Lpn Home Health Nurses Must Be Familiar With The Regulations And Guidelines Specific To Home Health Care. This Includes Knowledge Of Medicare/Medicaid Requirements, Osha Standards, And State Regulations For Home Health Agencies.

  • Compassionate And Patient-Centered Approach: Home Health Care Requires A Strong Focus On Patient-Centered Care, As Patients Are Often Recovering From An Illness Or Injury In The Comfort Of Their Own Home. Candidates Should Have A Compassionate And Empathetic Approach To Care, As Well As The Ability To Adapt To Different Patient Needs And Situations.
