You will be a part of an interdisciplinary team, collaborating with other professionals to ensure the best possible care for our patients and their families.The ideal candidate for this role will have a Master’s Degree in Social Work and be licensed in the state in which they are employed. In addition, we require a minimum of two years’ experience as a social worker, preferably in hospice or palliative care. The ideal candidate will also demonstrate excellent communication and interpersonal skills, as well as an unwavering dedication to providing quality care to our patients and their families. Responsibilities: -Provide emotional and practical support to patients facing serious illness, the end of life, and grief -Collaborate with other professionals to ensure the best possible care for patients and their families -Maintain a valid state license in social work -Maintain a minimum of two years of experience as a social worker -Demonstrate excellent communication and interpersonal skills -Provide quality care and compassionate service to patients and their families -Keep up to date with current best practices in social work -Work within the interdisciplinary team to ensure a holistic approach to care -Advocate for patients and their families -Maintain patient and family confidentiality -Provide resources to help patients and their families cope with serious illness and end-of-life situations
