As a Hospice Nurse Practitioner, you will have the opportunity to provide exceptional care and support to patients and their families during their end-of-life journey.   1. Provide compassionate and dedicated care to patients in hospice care.
  2. Work collaboratively with a team of healthcare professionals to develop and implement individualized care plans.
  3. Conduct thorough assessments of patients' physical, emotional, and spiritual needs.
  4. Administer medications and treatments as prescribed by hospice physician.
  5. Educate patients and their families on their condition, treatment plans, and end-of-life care options.
  6. Monitor and document patients' progress and make necessary adjustments to care plans.
  7. Provide emotional support and counseling to patients and their families.
  8. Coordinate patient care with other healthcare providers and community resources.
  9. Ensure compliance with all regulatory and company policies and procedures.
  10. Participate in ongoing education and training to maintain and improve clinical skills.
  11. Maintain accurate and timely documentation of all patient care.
  12. Communicate effectively with patients, families, and healthcare team members.
  13. Serve as an advocate for patients and their families, ensuring their needs and wishes are respected.
  14. Attend interdisciplinary team meetings and contribute to the development of patient care plans.
  15. Uphold the values and mission of PruittHealth in all interactions with patients, families, and colleagues.

Job Qualifications
  • Master's Degree In Nursing With Specialization In Hospice And Palliative Care.

  • Current Licensure As An Advanced Practice Registered Nurse (Aprn) In The State Of Tennessee.

  • Minimum Of 2-3 Years Of Experience In Hospice Or Palliative Care As A Nurse Practitioner.

  • Certification As A Hospice And Palliative Care Nurse Practitioner (Achpn) Preferred.

  • Strong Communication And Interpersonal Skills, Along With The Ability To Work Collaboratively With Interdisciplinary Teams.
