Hospice Community Relations Representative

We are currently seeking an experienced and compassionate Hospice Community Relations Representative to join our team. This position will be responsible for cultivating relationships with local hospice providers, securing referrals, and developing partnerships. The ideal candidate will possess excellent communication skills, the ability to work both independently and in a team setting, and a desire to make a difference in the lives of our patients and their families. The successful candidate must possess a Bachelor’s degree, a minimum of two years of experience in hospice or home health, and a valid driver’s license. Responsibilities:

1. Establish and maintain relationships with local hospice providers.
2. Secure referrals from hospice providers.
3. Develop partnerships with local businesses and organizations.
4. Build and foster relationships with patients and their families.
5. Represent PruittHealth at local events and meetings.
6. Assist in marketing and outreach efforts to promote the hospice program.
7. Facilitate communication between hospice providers and the PruittHealth team.
8. Maintain accurate records and reports related to referrals and relationships.
9. Keep up-to-date with industry trends and regulations.
10. Participate in professional development activities to stay informed of best practices.
