Case Mix Coordinator LPN

As a Case Mix Coordinator LPN, you will join the PruittHealth family in providing outstanding care and services to our residents. Responsibilities:

1. Ensure that all residents’ needs are met in accordance with the facility’s standards and regulations.
2. Monitor and evaluate resident care plans and adjust as necessary.
3. Assist in developing and implementing systems for documenting and tracking resident care provided in the facility.
4. Maintain accurate documentation of resident care and changes in condition.
5. Monitor and maintain documentation of resident care and services provided.
6. Act as a resource to staff and other healthcare professionals regarding resident care.
7. Communicate with members of the interdisciplinary team on a regular basis.
8. Participate in Quality Assurance and Performance Improvement (QAPI) activities in order to maintain high standards of care.
9. Collaborate with the Nursing Director to ensure compliance with facility policies and procedures.
10. Provide information and resources to residents and their families regarding available services and care options.
