You will play a vital role in addressing the social and emotional needs of our patients, promoting overall wellness and improving health outcomes.To be successful in this position, you must have a Master's degree in Social Work and be licensed in the state in which you will be practicing.   1. Provide comprehensive behavioral health services to patients in a primary care setting.
  2. Collaborate with primary care providers and other healthcare professionals to create a patient-centered treatment plan.
  3. Address the social and emotional needs of patients, including mental health and substance abuse disorders.
  4. Utilize evidence-based practices to effectively treat and support patients.
  5. Conduct assessments and develop individualized treatment plans for patients.
  6. Provide crisis intervention and support to patients in need.
  7. Maintain accurate and thorough clinical documentation.
  8. Advocate for patients and assist them in accessing appropriate resources and services.
  9. Collaborate with community organizations and agencies to enhance patient care and support.
  10. Participate in team meetings and provide input on patient care.
  11. Stay updated on industry trends, best practices, and regulations related to social work in a healthcare setting.
  12. Adhere to ethical standards and maintain confidentiality in all aspects of the job.
  13. Build and maintain positive relationships with patients, colleagues, and other professionals.
  14. Continuously assess and evaluate the effectiveness of interventions and adjust treatment plans as needed.
  15. Act as a mentor and resource for other team members, including interns and students.
  16. Adhere to all policies and procedures of the organization.
  17. Attend trainings, conferences, and workshops to enhance knowledge and skills.
  18. Embrace and promote a culture of diversity, equity, and inclusion in the workplace.
  19. Provide support and education to patients and their families regarding mental health and wellness.
  20. Act as a positive representative of the organization and promote its mission and values.

Job Qualifications
  • Master's Degree In Social Work From An Accredited University

  • Active Licensure As A Licensed Clinical Social Worker (Lcsw)

  • Minimum Of 2 Years Of Experience In A Primary Care Or Behavioral Health Setting

  • Knowledge And Understanding Of Evidence-Based Practices And Interventions For Mental Health And Substance Abuse Disorders
