
Patient Navigator

- Support patients in managing their health conditions by providing education and resources.
- Adhere to patient confidentiality and privacy regulations at all times.
- Collaborate with community organizations and resources to provide additional support to patients.
- Act as a positive representative of the healthcare organization, promoting a patient-centered approach to care.
- Attend training and development programs to enhance skills and knowledge in patient navigation.

Excellent Communication And Interpersonal Skills.
Bachelor's Degree In A Healthcare-Related Field, Such As Nursing, Social Work, Or Public Health.
Minimum Of 2 Years Of Experience In Healthcare Navigation Or Care Coordination.
Knowledge Of Community Resources And Healthcare Systems.
Experience Working With Diverse Populations And Understanding Of Cultural Sensitivity In Healthcare.
