  • Serve as the primary point of contact for patients and their families, providing support and guidance throughout their healthcare journey
  • Utilize a thorough understanding of the healthcare system and available resources to assist patients in navigating various services and programs
  • Conduct thorough assessments of patients' needs and develop personalized care plans to address their specific healthcare concerns
  • Collaborate with healthcare providers and other members of the care team to ensure seamless coordination of care for patients
  • Maintain accurate and up-to-date records of patient interactions and progress in navigation services
  • Provide education and advocacy for patients, empowering them to make informed decisions about their healthcare
  • Stay informed about changes and updates in healthcare policies and regulations, and communicate relevant information to patients as needed
  • Identify and address any barriers to care that patients may face, and work to connect them with appropriate resources and support services
  • Participate in community outreach and engagement activities to promote health and wellness and increase awareness of available healthcare resources
  • Continuously evaluate and improve patient navigation processes to enhance the overall patient experience and outcomes.

Job Qualifications
  • Bachelor's Degree In Healthcare Administration, Nursing, Social Work, Or Related Field.

  • Minimum Of 2 Years Experience In A Healthcare Or Social Services Setting, Preferably In A Patient Navigation Or Care Coordination Role.

  • Knowledge Of Healthcare Systems, Insurance Plans, And Resources Available For Patients.

  • Strong Communication And Interpersonal Skills, With The Ability To Build Rapport And Establish Trust With Patients And Their Families.

  • Understanding Of Cultural Competency And Ability To Work With Diverse Populations.
