Job Description

We are currently seeking a Patient Care Coordinator for our Heart and Vascular department who shares our passion for improving the lives of others. As a Patient Care Coordinator, you will play a vital role in coordinating care for patients with heart and vascular conditions. Your strong communication and organization skills, along with your knowledge of heart and vascular care, will allow you to effectively support our patients and their families throughout their treatment journey.   1. Coordinate and manage care for patients with heart and vascular conditions, ensuring a smooth and efficient treatment journey.
  2. Serve as a primary point of contact for patients and their families, providing support and guidance throughout the treatment process.
  3. Utilize strong communication skills to effectively communicate with patients, families, and healthcare professionals to ensure coordinated and comprehensive care.
  4. Collaborate with the healthcare team to develop and implement individualized treatment plans for patients.
  5. Utilize knowledge of heart and vascular care to educate patients and families on treatment options, potential risks and benefits, and self-care techniques.
  6. Maintain accurate and up-to-date patient records and document all interactions and interventions.
  7. Work closely with insurance providers to obtain necessary authorizations and ensure coverage for treatments.
  8. Assist in scheduling appointments, procedures, and follow-up care for patients.
  9. Ensure compliance with all regulatory and organizational policies and procedures.
  10. Continuously monitor and evaluate patient outcomes to identify areas for improvement and make recommendations for process enhancements.
  11. Participate in quality improvement initiatives and contribute to the development of best practices.
  12. Serve as an advocate for patients and their families, addressing any concerns or issues and helping to resolve them.
  13. Maintain confidentiality and adhere to ethical standards in all interactions with patients and their families.
  14. Stay updated on advancements in heart and vascular care and attend relevant trainings and conferences to continually improve skills and knowledge.
  15. Embody the mission and values of Providence St. Joseph Health in all aspects of the role.

Job Qualifications
  • Bachelor's Degree In Healthcare Administration, Nursing, Or A Related Field.

  • Minimum Of 3 Years Of Experience In A Healthcare Setting, Preferably In Cardiology Or Cardiovascular Care.

  • Strong Knowledge Of Medical Terminology, Procedures, And Insurance Policies Related To Heart And Vascular Care.

  • Excellent Communication And Interpersonal Skills, With The Ability To Interact With Patients, Physicians, And Other Healthcare Professionals.

  • Experience With Electronic Medical Records And Scheduling Systems, As Well As Proficiency In Microsoft Office And Other Relevant Software.
