- Conducting thorough patient evaluations to determine palliative care needs and developing individualized treatment plans.
- Providing expert medical care and symptom management for patients with serious illnesses.
- Collaborating with interdisciplinary team members, including nurses, social workers, and chaplains, to provide comprehensive support to patients and their families.
- Communicating effectively with patients and their families to educate them about their illness and treatment options.
- Offering emotional support and counseling to patients and their families during difficult times.
- Continually assessing and adjusting treatment plans to ensure the highest quality of care for patients.
- Participating in regular team meetings to discuss patient progress and make recommendations for care.
- Staying up-to-date with advancements and best practices in palliative care to continuously improve patient outcomes.
- Maintaining accurate and thorough medical records and documentation.
- Collaborating with other healthcare providers to ensure seamless continuity of care for patients.
- Providing education and training to other healthcare professionals on palliative care principles and practices.
- Adhering to all ethical and legal standards in patient care and treatment.
- Demonstrating empathy, compassion, and sensitivity towards patients and their families.
- Participating in quality improvement initiatives to enhance the overall patient experience.
- Contributing to a positive and supportive work environment that values teamwork and collaboration.
Medical Degree: A Minimum Of A Doctor Of Medicine (Md) Or Doctor Of Osteopathic Medicine (Do) Degree From An Accredited Medical School Is Required For This Role.
Board Certification: Candidates Must Be Board Certified In Palliative Care By The American Board Of Medical Specialties (Abms) Or The American Osteopathic Association (Aoa).
Residency Training: Completion Of A Residency Program In Either Internal Medicine Or Family Medicine Is Required. Additional Fellowship Training In Hospice And Palliative Medicine Is Preferred.
State License: Candidates Must Hold A Valid, Unrestricted Medical License In The State Where They Will Be Practicing.
Experience: A Minimum Of 3-5 Years Of Experience In Palliative Care Is Preferred, Although New Graduates With Strong Clinical Skills And A Passion For Palliative Care May Also Be Considered. Experience In A Hospital Or Hospice Setting Is Highly Desirable.
