  1. Conducting thorough patient evaluations to determine palliative care needs and developing individualized treatment plans.
  2. Providing expert medical care and symptom management for patients with serious illnesses.
  3. Collaborating with interdisciplinary team members, including nurses, social workers, and chaplains, to provide comprehensive support to patients and their families.
  4. Communicating effectively with patients and their families to educate them about their illness and treatment options.
  5. Offering emotional support and counseling to patients and their families during difficult times.
  6. Continually assessing and adjusting treatment plans to ensure the highest quality of care for patients.
  7. Participating in regular team meetings to discuss patient progress and make recommendations for care.
  8. Staying up-to-date with advancements and best practices in palliative care to continuously improve patient outcomes.
  9. Maintaining accurate and thorough medical records and documentation.
  10. Collaborating with other healthcare providers to ensure seamless continuity of care for patients.
  11. Providing education and training to other healthcare professionals on palliative care principles and practices.
  12. Adhering to all ethical and legal standards in patient care and treatment.
  13. Demonstrating empathy, compassion, and sensitivity towards patients and their families.
  14. Participating in quality improvement initiatives to enhance the overall patient experience.
  15. Contributing to a positive and supportive work environment that values teamwork and collaboration.

Job Qualifications
  • Medical Degree: A Minimum Of A Doctor Of Medicine (Md) Or Doctor Of Osteopathic Medicine (Do) Degree From An Accredited Medical School Is Required For This Role.

  • Board Certification: Candidates Must Be Board Certified In Palliative Care By The American Board Of Medical Specialties (Abms) Or The American Osteopathic Association (Aoa).

  • Residency Training: Completion Of A Residency Program In Either Internal Medicine Or Family Medicine Is Required. Additional Fellowship Training In Hospice And Palliative Medicine Is Preferred.

  • State License: Candidates Must Hold A Valid, Unrestricted Medical License In The State Where They Will Be Practicing.

  • Experience: A Minimum Of 3-5 Years Of Experience In Palliative Care Is Preferred, Although New Graduates With Strong Clinical Skills And A Passion For Palliative Care May Also Be Considered. Experience In A Hospital Or Hospice Setting Is Highly Desirable.
