  1. Coordinate and facilitate daily operations of the transitional care unit.
  2. Ensure smooth and efficient patient flow within the unit.
  3. Maintain a high level of organization and attention to detail in all tasks.
  4. Assist with scheduling and coordinating patient appointments and procedures.
  5. Communicate effectively with healthcare team members, patients, and their families.
  6. Keep accurate and up-to-date records of patient information and medical charts.
  7. Monitor and order necessary supplies and equipment for the unit.
  8. Collaborate with other departments and teams to ensure a seamless patient experience.
  9. Provide excellent customer service to patients and their families.
  10. Maintain confidentiality of patient information and adhere to HIPAA regulations.
  11. Follow all hospital policies and procedures.
  12. Assist with administrative tasks such as data entry, filing, and answering phone calls.

Job Qualifications
  • High School Diploma Or Equivalent: A Health Unit Coordinator Should Have A Basic Education To Understand Medical Terminology, Communication Skills, And Computer Proficiency.

  • Certification Or Training In Health Unit Coordination: Many Employers Prefer Candidates With A Certification Or Training In Health Unit Coordination, Which Includes Coursework In Medical Terminology, Healthcare Systems, And Communication Skills.
