Fieldhead Hospital SWYT, Kirklees Mental Health Pharmacy Primary Care Team, working with 9 different primary care networks which includes 65 GP surgeries across Greater Huddersfield and North Kirklees. Duties include reconciling discharge medication of patients discharged from our inpatient wards, making sure GP’s have patients repeat template up to date so there is no break in treatment for patient, speaking to patient who are taking anti-depressants which are no longer prescribed due to their side effects like dosulepin and trimipramine to try and remove this from their repeat, or reduce dose or switch to safer alternative, also speaking to patients over the age of 65 who are taking promethazine for anxiety/agitation again to either stop, reduce or switch as there is an increased risks of impaired cognition (processes that take place in the brain e.g. thinking, attention, language, learning, memory and perception) and falls, Looking at STOMP (stopping over medication of people with a learning disability, autism or both with psychotropic medicines) this is about working with patients to make sure they get the right medicine when they need it, making sure patients get any other support they require which may mean they are less likely to need more medicine, helping patients to stay well and have a good quality of life. Running searches on GP records to find patients who maybe overprescribed medicines to review their notes and book appointments with pharmacists to review patients medication to see if it can be reduced or stopped and
an alternative method to help support patient where medication is not required. Also, updating patients GP record with any medication they receive for community mental health team like clozapine and an anti-psychotic depot. Produce monthly and 6 monthly reports on progress of work and present this to GP surgeries we work with, writing SOP’s for procedures on how to update patient records and carry out tasks.

Primary Care Network, Central North Leeds. Working as a pharmacy technician for 8 surgeries which include Alwoodley Medical Centre, Meanwood Health Centre, North Leeds Medical Practice, Oakwood Surgery, Rutland Lodge Medical Centre, Shadwell Medical Centre, The St Lane Practice and The Avenue surgery, duties included actioning MHRA & Central Alert System, creating search on SystmOne and EMIS for issue with particular drug and pass information on to clinical pharmacists to review medication and make change if necessary. Carried out steroid card alert, where I reviewed patients on oral steroid and if they met criteria under SOP a steroid card was issued, also, spoke to patients taking oral methotrexate making sure they are taking correct dose and confirm what day of the week methotrexate is taken and note that on their repeat. Dealing with out of stock items, updating clinical pharmacists with which drug is on manufacturers cannot supply list, changing non ETP compliant items to ETP compliant, look at surgeries prescribing policy and to streamline the way it is done across the PCN surgeries. Look at repeat requests and identify items which are not needed every month to bring down costs for the surgery. Helped with Covid clinics, distributing vaccines and consumables to each room.

Locum Role, Bradford Royal Infirmary QA Department - duties include final checking prepacked medicine, making sure unlicensed meds that were ordered are correct and not damaged, writing reports on new unlicensed medicines, final checking emergency boxes.

Locum Role, Fairfield General Hospital - Duties included Accuracy Checking and dispensing prescriptions, also working on the discharge team, going to ward and accuracy checking discharge prescriptions for patients who are leaving the hospital.

Working as a locum pharmacy technician and ACT one day a week at Brudenell Road Pharmacy.

Working as a pharmacy technician dispensing prescriptions from ward cards for inpatients, I also checked stocks on ward and replaced stock where necessary which I invoiced to that particular ward, I picked stock from store room and packed in a box ready for the porter to take. Ward rounds, which include checking patients medication which is in their bed side locker making sure it matches the medication card and reorder medication where necessary.

Locum work dispensing prescription for different treatments which include HIV, Transplant, Clozaril, Fertility and Vitaflo products. I also accuracy checked prescriptions.

Photo Developing, Processing camera films, digital photos for customers walking into pharmacy and also processing for other shops in the local area, also having to maintain photo machine which involved cleaning machine and draining old liquid used to print pictures and refill machine with correct liquids in each compartment, also tried to solve any faults that occured. July 2008, Started working in the dispensary, dispensing prescriptions, which include walk ins, repeat prescriptions and venalink boxes, preparing methadone, ordering repeat prescriptions, checking prescriptions to make sure all items ordered have been given by surgery, placing orders through broadband ordering and over the phone, dealing with customer, surgery queries, dealing with orders from surgery and research company which involved me pricing items, ordering items and invoicing, dealing with company representatives, stock control, dealing with specials companies e.g. Eaststone Specials, JNK, Idis, Huddersfield Royal Infirmary, ordering special items, training new
members of staff, pre-reg and work experience students, supervising pharmacy when locum present. Doing deliveries when driver away on holiday also, running wholesale business which included me taking orders for non drug tariff items, specials, I had to invoice all orders and package them all to be posted out to other pharmacies. Also, doing ACT checks on prescriptions dispensed by other members of staff, this included walk in’s, repeat prescriptions which have been collected by the pharmacy and also venalink boxes. From August 2013 I am working as a Locum Technician and ACT.
