As an Advanced Practice Nurse Practitioner (APNP) I would like to join a medical center that offers me a constructive and challenging work environment for enhancing my skills backed by RN, BSN, DNP, and Family Nurse Practitioner-BC and contribute in reaching optimal healthcare standards. I have over 500 clinical hours in Family Practice in which I have gained a strong foundation, skills and knowledge in providing the best care to patients. In addition to my clinical hours, I am currently practicing as a nurse practitioner doing health risk assessments, both virtually and in-home for about the past three years.

*   Conduct yearly health assessment, screening, physicals and special education assessments.

  

*   Performed standardized health assessment, review current and past medical history, current prescription and address possible health concerns.

  

*   Provide patient education, health promotion and illness prevention.

  

*   Perform head to toe assessment physical examination, depression screening, monofilament test, foot exam, peripheral arterial disease test, A1c point of care and urine test.

*   Provides patient/family education and counseling in the areas of health promotion, maintenance, and disease prevention

  

*   Performing physical assessments, diagnosis, and treatment, ordering, and interpreting diagnostic tests, patient education, and evaluation

Family Nurse Practitioner Student

Responsibilities
Phone triage and walk-in's with family medicine, pediatric and women health, urgent care visits, telephone encounters, fax, prior authorizations, medication refills, rooming patient, nurse office visits, educating and etc.
  
Accomplishments
I've learn to triage over the phone and how to handle the inner patients of Milwaukee by educating and doing the best I can to help my patient's understand by teaching in a simple and concise way.
