  1. Provide emotional support and counseling to patients and their families throughout the transplant process.
  2. Conduct thorough assessments to identify the social, emotional, and practical needs of patients and develop appropriate care plans.
  3. Collaborate with healthcare team members to coordinate and facilitate the transplant process, including evaluations, pre-transplant education, and post-transplant care.
  4. Connect patients and their families with community resources and support services to assist with financial, housing, and transportation needs.
  5. Serve as an advocate for patients and their families, ensuring their needs and concerns are addressed and respected.
  6. Maintain accurate and organized patient records, documenting all interactions and services provided.
  7. Participate in multidisciplinary meetings and provide input on patient needs and progress.
  8. Conduct ongoing follow-up with patients post-transplant to monitor their well-being and provide ongoing support.
  9. Stay up-to-date on transplant policies, procedures, and resources to ensure the best possible care for patients.
  10. Maintain confidentiality and adhere to ethical standards in all interactions with patients and their families.
  11. Collaborate with insurance companies and other agencies to assist with securing coverage and benefits for transplant patients.
  12. Continuously evaluate and improve social work practices and procedures to enhance the patient experience.
  13. Provide education and support to healthcare team members on social work practices and the role of the social worker in the transplant process.
  14. Maintain a positive and professional demeanor while representing Piedmont Healthcare and upholding its values and mission.
  15. Attend trainings, conferences, and workshops to continuously enhance knowledge and skills in the field of transplant social work.

Job Qualifications
  • Master's Degree In Social Work (Msw) From An Accredited University.

  • Minimum Of 2 Years Of Experience In A Healthcare Setting, Preferably With Transplant Patients.

  • Knowledge Of The Transplant Process, Including Organ Procurement, Patient Evaluation, And Post-Transplant Care.

  • Proficiency In Providing Counseling, Support, And Resources To Patients And Families Dealing With Complex Medical Conditions.

  • Strong Communication And Collaboration Skills To Work Effectively With A Multidisciplinary Team, Including Physicians, Nurses, And Other Healthcare Professionals.
