- Develop and implement personalized nutrition plans for patients with Advanced Heart Failure undergoing transplantation.
- Conduct comprehensive nutritional assessments to determine patients' specific dietary needs and requirements.
- Collaborate with the interdisciplinary team to develop and implement individualized patient care plans.
- Provide education and counseling to patients and their families on proper nutrition, dietary modifications, and lifestyle changes.
- Monitor patients' progress and adjust nutrition plans as needed to ensure optimal outcomes.
- Stay up-to-date with the latest research and developments in the field of transplant nutrition to provide evidence-based care.
- Communicate with physicians, nurses, and other healthcare professionals to coordinate care and ensure a multidisciplinary approach to patient treatment.

- Adhere to all regulatory and safety standards related to nutrition and patient care.
- Participate in quality improvement initiatives and contribute to the development of best practices for transplant nutrition.

- Serve as a resource and educator for colleagues and staff on transplant nutrition.
- Attend conferences, seminars, and educational events to enhance knowledge and skills in transplant nutrition.

Bachelor's Or Master's Degree In Nutrition, Dietetics, Or Related Field From An Accredited Institution.
Registered Dietitian Nutritionist (Rdn) Certification From The Commission On Dietetic Registration (Cdr).
Minimum Of 2-3 Years Of Experience In Clinical Nutrition, Preferably In An Acute Care Setting.
Advanced Knowledge And Experience In Managing Diets For Patients With Advanced Heart Failure And Experience In Pre- And Post-Transplant Nutrition.
Strong Communication And Interpersonal Skills, With The Ability To Work Collaboratively With A Multidisciplinary Team And Educate Patients And Families On Complex Nutrition Needs.
