We are currently seeking a compassionate and highly skilled Sarcoma Navigator (RN) to join our team. As a Sarcoma Navigator, you will play a crucial role in guiding patients through their sarcoma journey, providing support, education, and coordination of care.   1. Serve as the primary point of contact for patients diagnosed with sarcoma, providing support, education, and guidance throughout their treatment journey.
  2. Coordinate and facilitate communication between patients, caregivers, and healthcare team members to ensure comprehensive and personalized care.
  3. Conduct thorough assessments of patients' physical, emotional, and psychosocial needs to develop individualized care plans.
  4. Provide patient and family education on sarcoma and treatment options, including potential side effects and self-care strategies.
  5. Collaborate with physicians and other healthcare professionals to develop and implement treatment plans and monitor patient progress.
  6. Maintain accurate and updated patient records, including treatment plans, progress notes, and other documentation as required.
  7. Ensure timely and appropriate referrals to other healthcare providers and community resources as needed.
  8. Stay up-to-date on the latest developments in sarcoma treatment and care to provide patients with the most current information and resources.
  9. Advocate for patients' needs and preferences to promote a patient-centered approach to care.
  10. Participate in multidisciplinary team meetings and contribute to the development and implementation of quality improvement initiatives.
  11. Demonstrate excellent communication and critical thinking skills to effectively communicate with patients, families, and healthcare team members.
  12. Adhere to all relevant healthcare regulations, policies, and procedures to ensure patient safety and confidentiality.
  13. Act as a resource and mentor for other healthcare team members, providing guidance and support as needed.
  14. Maintain a positive and professional attitude, promoting a collaborative and supportive work environment.
  15. Continuously evaluate and improve upon navigation services to enhance the patient experience and outcomes.

Job Qualifications
  • Bachelor's Degree In Nursing: A Sarcoma Navigator At Piedmont Healthcare Must Have A Bachelor's Degree In Nursing From An Accredited Institution. This Degree Provides A Strong Foundation In Nursing Theory And Clinical Practice, Preparing The Candidate To Provide High-Quality Care To Sarcoma Patients.

  • Active Rn License: A Sarcoma Navigator Must Have An Active Rn License In The State Of Georgia. This Ensures That The Candidate Has Met The Standards Set By The State For Nursing Practice And Is Qualified To Provide Patient Care.

  • Clinical Experience: Candidates For The Sarcoma Navigator Role Must Have A Minimum Of 2-3 Years Of Experience Working In Clinical Settings, Preferably In Oncology Or A Related Field. This Experience Provides A Strong Understanding Of Cancer Care And Treatment, Which Is Essential For Navigating Patients Through Their Sarcoma Journey.

  • Oncology Certification: A Candidate With An Oncology Certification, Such As The Ocn (Oncology Certified Nurse) Or Aocn (Advanced Oncology Certified Nurse) Is Highly Desirable For The Sarcoma Navigator Role. This Certification Demonstrates A Specialized Knowledge And Skillset In Cancer Care, Which Is Crucial For Providing Comprehensive Support To Sarcoma Patients.

  • Excellent Communication And Interpersonal Skills: As A Sarcoma Navigator, The Candidate Must Have Exceptional Communication And Interpersonal Skills To Effectively Engage With Patients, Families, And Healthcare Teams. This Includes The Ability To Provide Emotional Support, Educate Patients About Their Condition, And Collaborate With Other Healthcare Professionals For Optimal Patient Care.
