Registered Nurse - Care Manager

1. Provide direct patient care by assessing, planning, implementing, and evaluating patient needs and developing individualized care plans.
2. Collaborate with interdisciplinary healthcare team members to ensure comprehensive and coordinated care for patients.

11. Identify and address any potential barriers to care and find solutions to improve patient outcomes.
12. Collaborate with insurance providers and other organizations to ensure appropriate and timely care for patients.
13. Continuously monitor and evaluate patient outcomes to identify areas for improvement and implement appropriate changes.
14. Maintain a professional and courteous demeanor at all times, providing excellent customer service to patients and their families.
15. Participate in performance improvement initiatives to enhance the quality of care provided to patients.
