  9. Identify and address potential issues or concerns related to patient care and take appropriate action.
  10. Educate patients and their families on treatment plans, medication management, and other healthcare topics.
  11. Stay updated on industry developments and trends to continuously improve care management strategies.
  12. Adhere to all ethical and legal standards in patient care.
  13. Collaborate with other healthcare facilities and organizations to coordinate care for patients.
  14. Provide support and guidance to less experienced care management staff.
  15. Uphold the mission and values of Piedmont Healthcare in all job duties.

Job Qualifications
  • Bachelor's Degree In Nursing Or Related Healthcare Field.

  • Experience With Electronic Medical Records And Proficiency In Computer Skills.

  • Minimum Of 3-5 Years Of Experience In A Healthcare Setting, Preferably In Case Management Or Care Coordination.

  • Strong Knowledge Of Healthcare Regulations, Insurance Coverage, And Reimbursement Processes.

  • Excellent Communication And Interpersonal Skills, With The Ability To Collaborate With Healthcare Teams And Effectively Communicate With Patients And Their Families.
