- Coordinate and manage the care of patients in accordance with Piedmont Healthcare's mission and standards of care.
- Develop and implement personalized care plans for each patient in collaboration with the healthcare team.
- Conduct comprehensive assessments of patients' physical, emotional, and social needs.
- Communicate with patients and their families to ensure they understand the care plan and are involved in decision-making.
- Monitor patients' progress and make necessary adjustments to the care plan as needed.
- Collaborate with physicians, nurses, and other healthcare professionals to ensure the best possible outcomes for patients.
- Stay up-to-date on the latest developments and best practices in healthcare to provide the highest quality of care to patients.
- Advocate for patients' needs and ensure they receive appropriate resources and support.
- Document all patient interactions, care plans, and progress notes accurately and in a timely manner.
- Participate in team meetings and case conferences to discuss patients' progress and develop effective care strategies.
- Maintain confidentiality of patient information and adhere to all HIPAA regulations.

- Continually strive to improve the quality of care provided by identifying areas for improvement and implementing appropriate solutions.

Excellent Communication And Interpersonal Skills.
Bachelor's Degree In Healthcare Management, Nursing, Or A Related Field.
Minimum Of 3 Years Experience In A Healthcare Setting, Preferably In A Managerial Role.
Strong Understanding Of Healthcare Regulations And Compliance.
