  1. Coordinate and manage the care of patients in accordance with Piedmont Healthcare's mission and standards of care.
  2. Develop and implement personalized care plans for each patient in collaboration with the healthcare team.
  3. Conduct comprehensive assessments of patients' physical, emotional, and social needs.
  4. Communicate with patients and their families to ensure they understand the care plan and are involved in decision-making.
  5. Monitor patients' progress and make necessary adjustments to the care plan as needed.
  6. Collaborate with physicians, nurses, and other healthcare professionals to ensure the best possible outcomes for patients.
  7. Stay up-to-date on the latest developments and best practices in healthcare to provide the highest quality of care to patients.
  8. Advocate for patients' needs and ensure they receive appropriate resources and support.
  9. Document all patient interactions, care plans, and progress notes accurately and in a timely manner.
  10. Participate in team meetings and case conferences to discuss patients' progress and develop effective care strategies.
  11. Maintain confidentiality of patient information and adhere to all HIPAA regulations.
  12. Uphold Piedmont Healthcare's values of compassion, integrity, teamwork, and excellence in all interactions with patients, families, and colleagues.
  13. Continually strive to improve the quality of care provided by identifying areas for improvement and implementing appropriate solutions.

Job Qualifications
