We are a premier healthcare system in Louisville, Kentucky, and offer a wide range of services to our patients. In this role, you will be responsible for providing care coordination and transition management services to patients and their families. This is a PRN position, with flexible hours and the ability to work remotely.To be successful in this position, you must have a minimum of two years of experience in case management, discharge planning, care coordination, or a related field.
